This privacy policy, as part of the consent form, sets out how Aimee McEvilly uses and protects any personal information that you provide us.

 

Aimee McEvilly is committed to ensuring that your privacy is protected. Should she ask you to provide certain information by which you can be identified when using her services, you can be assured that it will only be used in accordance with this privacy policy. 

This policy is effective as from March 2024 and will be regularly reviewed and updated.

Why collect your data?

 

Your personal data is collected because you give us consent to do so, in order to provide independent nutrition and dietetic services to you. 

What data is collected?

 

The following information may be collected: 

  • Full name, occupation, gender and marital status

  • Contact information and personal details – home address, email, contact telephone numbers, details of GP and other medical professionals

  • Current and past medical history, current medications, body measurements and lifestyle information provided by the client

  • Further clinical information in relation to reports from other health professionals

What is done with the information gathered:

 

This information is required for the purpose of: 

  • Professional clinical record keeping of client information. 

  • Sharing information with relevant health professionals. 

Security

 

I am committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, I have put in place encrypted electronic systems and standard operating procedures to safeguard and secure the information collected. 

Controlling your personal information

I will not distribute, sell or lease your personal information to third parties unless given your explicit permission or are required by law to do so.

 

You may request details of personal information which we hold about you under the General Data Protection Regulation (GDPR).

If you believe that any information I am holding on you is incorrect or incomplete, please email or write to me as soon as possible at the above address. I will promptly correct any information found to be incorrect.

 

How you can withdraw and request to be deleted from our files

Disclosure of your information


You agree that we have the right to share your Personal Data with:

  • Relevant health professionals (only when consent is given by the client).

  • In exceptional circumstances information about a client may be disclosed without consent if it is in the public interest to do so. This might be in circumstances where disclosing the information is necessary to prevent a serious crime or serious harm to other people.

  • If we are under a duty to disclose or share your Personal Data to comply with any legal obligation, or to enforce or apply our terms of business or terms of use, and other agreements; or to protect the rights, property, or safety of our business, our clients, or others.
